Office Policies

Clients may look at, print, and fill out the following forms prior to our first visit. Please click on the document’s name to view or download the files and then fill out and sign the forms before we meet for the first time. Couples should download and complete his or her own Client Information Form and then both read, sign, and date the Office Polices Form and HIPPA notice.
